Definition of Aftercare
The time period immediately following departure from an addiction treatment program and facility is referred to as aftercare. This time is used to get individuals back on the right track. At Prominence Treatment Center, helping individuals get their lives back on track is the ultimate goal. Discharge specialists along with primary therapists work together to assist individuals in structuring the most effective plan for future sobriety.
Aftercare from Drug Rehab
The conclusion of a rehabilitation treatment program is just the start to successful recovery from drug and alcohol addiction. Unfortunately the consequences of drug abuse cannot be wiped away. In short, there is no ultimate cure for addiction, just continued treatment and awareness of the problem. There is however, something to look forward to, something to give hope. Prominence Treatment center offers continuing support and care even after individuals leave our facility. Our aftercare programs and discharge planning both play crucial roles in preventing relapse and preserving abstinence.
Is aftercare important?
The days, weeks and months following treatment are very difficult. Many individuals become overwhelmed by the day to day activity they return to and often times they lose their ability to function. By speaking with someone who understands what you went through you can advance yourself identity and comfort with the seemingly new world. In whole, the longer that you maintain contact with treatment advisors the more likely you are to stay true to what was learned through recovery.
Choices for aftercare can include one or all of the following:
Outpatient counseling
Weekly phone or Skype check-ins & meetings
Finding the proper recovery & relapse prevention groups
Before an individual completes their time with Prominence they will have met with our aftercare specialist. Together with the specialist, individuals map out a successful guide to staying clean and sober. Our specialist will work to ensure that every individual is put in contact with a therapist, doctor or support group that would build on their achievements at Prominence and enhance their recovery at home. As part of our program we will work tirelessly to ensure that your support team is made aware of your progress and, with your approval, takes part in your aftercare planning.
Most importantly, we never stop caring. Every week, for one year after you are discharged, our designated staff member will contact each individual to make sure that they are still working towards staying clean and sober. Each individual that takes part in any of Prominence Treatment Center's many treatment programs is automatically part of the Prominence Alumni. As an alumnus you will receive news and notifications about our programs and our alumni retreats and programs.
By the end of your stay with Prominence our hope is that you are ready and able to live a clean and sober and healthy life at home. We will do everything that we can to help you succeed.
Aftercare services are provided for 18 months following the completion of any of Prominence’s treatment programs.
